Care-team and patient-placement data must be kept accurate for downstream scheduling, routing, and quality programs, and maintaining it manually is repetitive and error-prone. Each quarter the health plan or system issues an attributed-provider file assigning patients to a primary care physician, and every patient's Epic Care Team PCP has to be updated to match it. Reconciling thousands of records by hand is slow, and when the Care Team PCP drifts out of sync with attribution, scheduling, routing, and quality-program credit all key off the wrong provider.
