Problem Statement

When a patient is being discharged to post-acute care, the care management team works with them to choose a placement, and that selection has to land in the patient's Epic chart for the care team to act on. When the choosing happens in a separate platform like Repisodic, the information has to cross from that system into Epic, and if it's transferred by hand the step is slow and easy to drop in the middle of a busy discharge. A placement that never makes it into the chart leaves the care manager working without it and stalls the patient's transition of care, so the handoff has to be reliable and the care manager has to know immediately if it isn't.

Automation Solution

This automation receives post-acute placement information from Repisodic via an API call and enters it into the patient's Epic chart; if the entry fails, it notifies the assigned care manager(s) by TigerText so the placement isn't silently lost.
