Problem Statement

When medical records are submitted in response to an additional documentation request, the result of that submission has to be captured against the account so the denial documentation stays current. When this update lags or is skipped, the account doesn't show that the ADR was satisfied, so staff can't tell which open denials have already been worked, leading to duplicated effort, re-submissions, or accounts that stall because no one can see their true status.
