Problem Statement

Care-team and patient-placement data must be kept accurate for downstream scheduling, routing, and quality programs, and maintaining it manually is repetitive and error-prone. Behavioral Health providers have to be added to (and removed from) the right patients' Epic Care Teams, because scheduling and medication workflows route based on Care Team membership. When that membership is wrong or stale, visits and medication orders route to the wrong provider, so the placement has to be kept current as assignments change.

This automation identifies Behavioral Health providers via an Epic report and adds or removes them from Epic's patient Care Teams so scheduling and medication workflows route correctly.
