Problem Statement

Care teams for dual-eligible members can only coordinate benefits and services when they have the member's Medicare identifiers and dual-eligibility details in hand, but that information lives in a separate state provider portal and has to be looked up and passed along per member. Retrieving it manually for every Community Health Choices member is slow, repetitive lookup work that delays the social workers and care teams who need it.

Automation Solution

This automation retrieves the Medicare Recipient ID and Dual Eligibility Special Needs Plan (DSNP) Community Health Choices information from the PROMISe provider portal to share with Community Health Choices social workers and care teams.
